Adults (18+)
This is the referral form that should be used if you are aged 18 years or over and would like to refer yourself to the Brighton and Hove Wellbeing Service. You can make a referral straight to us this way any time of day or night. You must either be living in Brighton & Hove, or registered with a GP in the area.

The information you provide is confidential and we use it to help understand how we might best meet your needs.

Children & Young People (Under 18)
Counselling & other face-to-face support
This referral form should be used if you would like to refer yourself and you are aged 13-17 years old, or if you are an adult who would like to refer a child or young person aged 4-17 years old.
This form can be filled out by a young person aged 13-17 years old, a parent/carer, a teacher, or other professional wanting to refer a child or young person aged 4-17 years old.
If you are aged under 13 years old, you will need permission from a parent/carer first.
The form will ask you what you to describe what would like help with: what the main issue is, how long it has been happening, and how it affects everyday life.
When the form gets sent to us, the Children & Young People’s team will review the information and then contact you to let you know the outcome.
To help us make a decision, we may also discuss this information with our colleagues in other services e.g. Schools Wellbeing mental health service or specialist Child and Adolescent Mental Health Services (CAMHS).
