This is the referral form that should be used if you are aged 18 years or over and would like to refer yourself to the Brighton and Hove Wellbeing Service. You can make a referral straight to us this way any time of day or night. You must either be living in Brighton & Hove, or registered with a GP in the area.
If you have any queries, or if you are unable to complete an online self-referral form, please call our admin team on 0300 002 0060 for assistance. Our phonelines are open Monday-Friday, 8am-6pm.
The information you provide is confidential and we use it to help understand how we might best meet your needs.
Brighton & Hove Wellbeing Service/Health in Mind is part of Sussex Partnership Foundation Trust. We work closely with specialist teams within the trust to ensure that individuals get the right care as soon as possible. We work with the Neighbourhood Mental Health teams who bring together local NHS mental health services, GPs, and community and voluntary organisations to offer joined-up, personalised support for your mental health.
If we think that a specialist service within this team would suit your needs better we would consider transferring your referral. If you have a booked assessment or you are seeing a member of our team regularly they would discuss this with you and agree next steps.
